How to access our services
Find out how you or a family member can be referred to Katharine House Hospice.
Referrals to the inpatient and community care services
If you feel that you need extra help and would like to access our services, talk to your GP or hospital consultant.
If they are in agreement, they will then contact Katharine House Hospice on your behalf. Only then can we contact you directly. Please note that in all cases, you (the patient) must agree to the referral.
If you’re registered with any of the following GP surgeries, we will be able to accept your referral, as long as your GP gives consent.

Self-referring to Living Well
New referrals are welcomed by the Living Well team. You may self-refer or a healthcare professional can refer on your behalf.
To self-refer, contact us to arrange an initial assessment to establish your suitability for the Living Well service. There are three ways to do this:

Palliative Care Hub
If you have been referred to the palliative care team and have worsening symptoms or need support, visit the Palliative Care Hub page on the OUH website where you will find contact details and information on when to contact the Hub and when to call elswhere.
This provision is also available for carers and others who are important to the patient if they have any concerns.
