Katharine House accepts referrals for patients with any advanced life-limiting illness from the following surgeries.

• Banbury surgeries: Hightown, Horsefair, West Bar, Windrush, Woodlands Health Centre
• Bloxham Surgery
• Brackley surgeries: Brackley Medical Centre, Springfield Surgery
• Byfield Surgery
• Charlbury Medical Practice
• Chipping Norton Health Centre
• Cropredy Surgery
• Deddington Health Centre
• Hook Norton Surgery
• Shenington Surgery
• Wychwood Surgery, Shipton Under Wychwood

Consideration is also given to patients from outside the area whose family is registered with one of these GP practices.

To refer a new patient to us, please complete the Living Well referral form, which is an editable pdf.

A palliative care on-call doctor is available via the OUH switchboard for advice to healthcare professionals outside normal working hours. Please call 0300 304 7777.
